ACKNOWLEDGEMENT & RELEASE OF LIABILITY FORM

I, the applicant, on behalf of myself, members of my family, my heirs, executors, administrators and assigns, hereby forever release, discharge and hold harmless Ground Control Academy, its heirs, successors, administrators, representatives, instructors, employees and agents for any injury, loss or damage to my person or property howsoever caused, arising out of or in connection with my taking part in Martial Arts Classes, Seminars and activities including the training and techniques taking place on the premises, from the instructor(s), staff, official or employees of the school or any fellow students on the premises in the connection herewith and within the course of taking training lessons for the purpose designed in this application and notwithstanding that the same may have been contributed to or occasioned by the negligence of Ground Control Academy, representatives or agents.

Please note:
Participants must supply their own protective equipment. The undersigned assumes
all risk inherent and incident of this type of sports activity as a condition for applying for admission to this school for the purpose hereinabove stipulated. If you have any medical problems, it is recommended that you consult a physician prior to taking this class. This is a high impact, high intensity class. Any person who has or is currently teaching/instructing any fitness or martial arts programs must identify themselves prior to class. Ground Control Academy reserves the right to deny entry. Any reproduction of this program in any form i.e. class material, curriculums, lesson plans etc is strictly prohibited. Legal action will be taken against individuals or organizations who attempt to replicate this program in any form.

The undersigned acknowledges that:

1. He/She is desirous of using, as a member on a membership basis, the Martial Arts School herein referred to as “Ground Control Academy”.

2. He/She has received a completely executed copy of this agreement.

3. He/She confirms that there were no verbal presentations other than those specified in this agreement.

4. He/She may be photographed or filmed while attending at the premises of Ground Control Academy and he/she gives permission to Ground Control Academy, and any affiliates to use any and all photos, film footage, and/or video streaming for promotional, sales and advertising purposes for all media including internet.

5. The waiver was read, understood and he/she agrees to abide by it.
